Delivers patient care by incorporating the tenets of a professional practice model of Relationship Centered Care and applies the nursing process based on theory, research, and evidence-based practice. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning after hospital care, delegating to others as appropriate. Educates patients and caregivers about their plan of care, transition of care, health promotion, and disease prevention while anticipating needs and readiness to learn. Communicates effectively and professionally with patients, families, and all members of the interdisciplinary patient care team. Manages assignments within the care delivery model and demonstrates effective allocation of material resources. Role models relationship-centered care through the facilitation of various patient care activities. Establishes and maintains a therapeutic relationship with patients and families. Uses evidence-based practices to enhance understanding of patients' perceptions of care. Initiates service recovery for patients and families with concerns and escalates to appropriate management as needed. Demonstrates caring, compassion, empathy, and active listening in interactions with patients and families. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety for the patient population served. Implements process improvement strategies based on evidence-based practices. Contributes to quality and safety practices and required compliance measures. Documents in an accurate and thorough manner in compliance with regulatory and legal requirements and standards of care. Coordinates with management to ensure supplies are utilized appropriately and inventory is sufficient for patient care. Performs charge nurse duties as assigned. Encourages a spirit of scholarship, inquiry, and life-long learning for self and others. Participates in shared decision-making through nursing department committees and unit initiatives.
