The RN Case Manager is responsible for providing nursing care to patients, including identifying and coordinating patient / family care with other members of the Interdisciplinary Team and health care providers.

RESPONSIBILITIES :

• Completes an initial assessment of patient and family to determine hospice care needs; provides a complete physical assessment and history of current and previous illness
• Uses health assessment data to determine nursing diagnosis
• Triages calls and assists patient / primary caregiver with problem resolution
• Teaches and counsels patients and families in meeting nursing and related needs
• Makes visits for symptom management and support as needed
• Modifies patient treatment plans as indicated by patients' responses and conditions
• Maintains current, accurate, detailed reports in accordance with policy
• Monitors, records and reports symptoms and changes in patients' condition to the IDG
• Provides health care instructions to the patient as appropriate per assessment and plan
• Obtains and receives physician orders as required
• Pronouncement of patients
