Physical Therapist – Home Health / Outpatient

We are seeking a dedicated and compassionate Physical Therapist to join our home health team, providing high-quality care to patients in the comfort of their own homes. Key Responsibilities :

Perform thorough assessments to determine each patient’s physical therapy needs.

Develop and implement individualized treatment plans designed to improve strength, mobility, and overall physical function.

Administer therapeutic exercises and techniques tailored to patient-specific goals.

Educate patients and their families on home exercise programs and strategies to maintain progress.

Collaborate with other healthcare providers to ensure coordinated, holistic care.

Maintain accurate documentation of patient progress and update treatment plans as needed to optimize outcomes.

Qualifications :

Graduation from an accredited Physical Therapy program.

Valid state license to practice as a Physical Therapist.

Minimum of one year of experience in home health or a related healthcare setting preferred.

Strong interpersonal and communication skills with a compassionate, patient-centered approach.

Ability to work independently and manage a flexible schedule.

Commitment to providing high-quality care and improving patient outcomes.
