Location : Various home throughout the Central Richmond Area requiring some travel.

Position Overview
We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) to join our ID / DD Services Department. This role carries primary medical responsibility for individuals across all ID / DD programs, ensuring high-quality care, compliance with DBHDS and Human Rights standards, and strong collaboration with staff, families, and external providers. The LPN will play a critical role in clinical oversight, staff training, and risk management within the agency.
Key Responsibilities
- Conduct annual and ongoing nursing assessments for all individuals served.
- Provide direct clinical support, including administering medications, injections, and treatments as prescribed.
- Ensure each individual is seen at least once monthly, documenting a nursing note for the medical record.
- Manage and track all medical appointments, ensuring individuals attend and receive necessary follow-up care.
- Communicate and advocate with physicians, specialists, and medical care teams to ensure the best outcomes for individuals.
- Assist individuals with activities of daily living (ADLs) when necessary to support health and wellness.
- Oversee dietary needs and health expectations, including monitoring and supporting individuals with diabetes management.
- Provide support during hospital visits, admissions, and discharges to ensure continuity of care.
- Serve as a member of the agencys on-call rotation and Risk Management Team.
- Conduct medication administration record (MAR) audits and nursing audits for compliance and accuracy.
- Deliver on-site staff training and refreshers on medication management.
- Provide and track staff certification in CPR and First Aid, ensuring compliance with annual / biannual deadlines.
- Partner with leadership to promote health and wellness across all ID / DD programs.
Schedule Flexibility
We understand the importance of work-life balance and are open to flexible scheduling arrangements. Options such as four 10-hour days with one day off or other negotiated schedules can be discussed during the hiring process to best meet both agency and employee needs.
Qualifications
Current, active LPN license in Virginia (required).Minimum 2 years of nursing experience, preferably in ID / DD services, residential care, or related healthcare settings.Strong knowledge of DBHDS regulations, Human Rights, and Medicaid requirements (preferred).Excellent organizational, communication, and training skills.Ability to manage multiple priorities and maintain thorough documentation.Commitment to person-centered, compassionate care.What We Offer
