Description :

This position requires a Case Manager RN with a minimum of 1 year of inpatient care management experience.

The role will involve floating between units as needed.

On-call and weekend shifts are not required.

Expect patient ratios of 1 : 18.

The Case Manager will focus on discharge planning rather than utilization review.

Interdisciplinary support will be available from social services.

Candidates should have skills in care coordination, discharge planning, and prior authorizations.

Travelers may float to other facilities based on skill sets.

Weekly working hours will be guaranteed, and time off requests must be discussed up front.

Compliance with scheduled shifts is mandatory.

A vehicle is required for this assignment.
