Job description

American Traveler is seeking an experienced RN with active CA license and recent case management experience for a Home Health assignment.

Job Details

Home Health setting providing care to patients in their homes,
Day shift schedule with five 8-hour shifts from 8 : 00am to 5 : 00pm,
Assignment duration is approximately 13 weeks,
Applicants must reside at least 50 miles from the facility to qualify for travel status,

Job Requirements

Active California RN license in hand,

Current BLS certification required,

Valid CA driver's license and auto insurance required,

Minimum 2 years of recent relevant nursing experience,

Previous case management experience is mandatory,

Additional Information

Responsible for assessment, coordination, delivery, and evaluation of in-home nursing care,

Direct patient care, patient / family education, and effective care transitions are required,

Develop and implement individualized, interdisciplinary plans of care factoring in psychological, sociocultural, and lifestyle elements,

Engage in collaboration with patients, families, physicians, and the healthcare team,

Adherence to established policies, procedures, and industry standards is expected,

Only one holiday request off (RTO) will be approved during the assignment,

Returning staff must have not worked for this system in the past year to be eligible,

Must be available to complete lab work within 3-5 days of contract signing,
