Job description

American Traveler is seeking an experienced RN with active CA license and recent case management experience for a Home Health assignment.

Job Details

  • Home Health setting providing care to patients in their homes,
  • Day shift schedule with five 8-hour shifts from 8 : 00am to 5 : 00pm,
  • Assignment duration is approximately 13 weeks,
  • Applicants must reside at least 50 miles from the facility to qualify for travel status,

Job Requirements

  • Active California RN license in hand,
  • Current BLS certification required,
  • Valid CA driver's license and auto insurance required,
  • Minimum 2 years of recent relevant nursing experience,
  • Previous case management experience is mandatory,

    • Additional Information

  • Responsible for assessment, coordination, delivery, and evaluation of in-home nursing care,
  • Direct patient care, patient / family education, and effective care transitions are required,
  • Develop and implement individualized, interdisciplinary plans of care factoring in psychological, sociocultural, and lifestyle elements,
  • Engage in collaboration with patients, families, physicians, and the healthcare team,
  • Adherence to established policies, procedures, and industry standards is expected,
  • Only one holiday request off (RTO) will be approved during the assignment,
  • Returning staff must have not worked for this system in the past year to be eligible,
  • Must be available to complete lab work within 3-5 days of contract signing,
