Job Description

Job Title : Physical Therapist

Must-Have :

  • Home health experience with a minimum of 2 years of clinical experience

Job Description :

  • Provide exceptional and personalized care to patients in a home health setting.
  • Conduct home health admissions, re-certifications, and functional assessments.
  • Supervise Physical Therapist Assistants (PTA) and manage all patient physical therapy needs, including communication with medical offices.
  • Perform 5-6 home visits per day in various areas of McDowell County, NC.
  • Use equipment such as laptops and iPhones; utilize electronic medical records through Cerner Roadnotes.
  • Maintain an autonomous, motivated, self-directed, and organized approach to patient care.
  • Manage a caseload for PT and PTA, with a minimum of 5.6 visit equivalents per day.
  • Participate in a weekend on-call rotation every 4-6 weeks, covering day shift visits, evaluations, functional assessments, or admissions.

    • Please adhere to scrub color or attire requirements as specified by the unit guidelines or policies.
