Overview

Responsibilities and qualifications below reflect the role of an LPN in a Family Practice clinic setting, including patient care under supervision, documentation, and coordination with providers and care teams.

Responsibilities

• Assesses and implements patient care under supervision. Prepares patient for examination.
• Provides direct patient care. Assists provider with examination and treatment of patient as directed.
• Under provider oversight, may be responsible for telephone triage using age-specific triage protocol books and computerized resources.
• Documents patient care in health records according to department standards.
• Obtains and conveys information to and from patients, pharmacies, and other providers as directed and supervised by a licensed provider. May obtain referral authorizations for treatment as directed by provider.
• Ensures ongoing professional development through continued education.
• Performs other duties as assigned.
