Full Time - First and Second Shifts Available

Pay Rate : $28.00 to $32.00 based on experience

Job Summary :

As a Behavioral Health Case Manager III, you will play a crucial role in our agency's efforts to provide comprehensive support and resources to individuals and families facing behavioral health challenges. In this senior-level position, you will leverage your extensive experience and expertise to coordinate care, advocate for clients, and ensure they have access to the services and support they need to thrive.

Key Responsibilities :

• Conduct comprehensive assessments to identify clients' needs and develop personalized care plans
• Coordinate and facilitate access to a range of behavioral health services, including counseling, therapy, and medication management
• Collaborate with a multidisciplinary team of healthcare professionals to ensure seamless, coordinated care
• Advocate for clients' rights and needs, and navigate complex systems to secure appropriate resources and support
• Provide ongoing case management, monitoring, and follow-up to ensure clients' progress and well-being
• Maintain detailed case records and documentation in accordance with agency policies and regulatory requirements
• Participate in team meetings, training, and professional development activities to continuously enhance your skills and knowledge
