Job descriptionConduct comprehensive patient evaluations, assessments, and follow-up visits for pain management . Evaluate potential patient profiles for acceptance to pain management Develop, implement and adjust individualized treatment plans, including medication management. Prescribe, monitor and evaluate the effectiveness of medications for pain management interventions. Collaborate with physicians and healthcare staff to ensure comprehensive patient care. Maintain accurate and up-to-date medical records and document patient progress. Provide education and support to patients and their families regarding pain management strategies and treatment options. Advocate for patients and help them navigate the healthcare system. Board certification as a Nurse Practitioner with an active Wisconsin license in a relevant specialty (Pain Management, Family Medicine, or Internal Medicine preferred). Minimum of 2 years of pain management experience preferred. Must possess a valid and active DEA license for prescribing controlled substances- ( required and non-negotiable). Must be eligible for In-network or order / prescribe / refer status with Wisconsin Forward Health (Clinic can assist with order / prescribe / refer application if necessary; Clinic does not bill insurance for patient visits, but must be active in Wisconsin Forward Health for prescriptions to be recognized). Knowledge of both pharmacological and non-pharmacological pain management techniques. Must demonstrate empathy and a patient-centered approach to care. Strong clinical judgment and decision-making skills. Excellent communication and interpersonal skills. Ability to work collaboratively in a multidisciplinary team. Strong documentation and patient care planning skills. Knowledge of healthcare regulations and standards. Proficiency with Electronic Health Record and Office 365. Commitment to continuing education and professional development. Comfortable in providing care to a largely urban population of varying ethnic, socioeconomic and cultural backgrounds. Full time position, 40-50 hours / week. Onsite : 4 days a week, 7 : 30am-5 : 00 pm. Reporting and documentation may be done remotely after clinic hours. No call, nights, weekends or holiday.
