Is responsible for maintaining a formal comprehensive diabetes education program encompassing medical / clinical providers, physical therapists, mental health practitioners, and health educators. Monitors equipment that provides eligible patients with the tools and support to self-manage their diabetes, thus reducing chronic care costs.
