Job Description
Analyzes medical records for deficiencies. Performs maintenance tasks to ensure completion of medical records.
Job Responsibility
- Conducts a daily medical records review to identify possible documentation deficiencies.
- Enters any medical record deficiencies into the computer system.
- Generates appropriate deficiency slip to indicate type of record deficiency, identifying un-dictated operative reports, D / C summaries, and missing signatures on discharge summaries, operative reports, consultations, and physician orders.
- Files all documents in appropriate chart order according to established procedure.
- Documents and / or updates incomplete chart information and assigns to appropriate physician for completion.
- Checks for operative reports and discharge summaries not yet placed in the medical record.
- Reviews incomplete charts to determine which record deficiencies have been corrected. Updates computer system to reflect the status of chart.
- Performs related duties, as required.

Job Qualification
