Melmark is a nonprofit organization providing clinically-sophisticated evidence-based special education, residential, vocational, and therapeutic services for children and adults diagnosed with autism spectrum disorders, developmental and intellectual disabilities, acquired brain injuries, medical complexities, and other neurological and genetic disorders.
LPN Overview
This position assumes the primary role in providing and monitoring the healthcare and health maintenance of clients assigned by Melmark for a designated nursing line. The Primary Nurse works collaboratively with other members of the multidisciplinary treatment team for case management. The Primary nurse is the key interface with Primary Care Providers, pharmacy, medical insurance companies and medical equipment providers. Must possess an ability to maintain effective communication with families, other nursing staff and the interdisciplinary team. The Primary Nurse is responsible for participation in client meeting including client reviews, IEP / IPP's, interagency meetings for the coordination of comprehensive medical care and services. Maintains oversight of client charts and excellence in quality of care through audits and regular monitoring.

- Starting Rate : $30.00 / hour (+) based on years of experience
- On call rotation and holidays as assigned

Current licensure as a Licensed Practical Nurse through the Pennsylvania Board of Nurse ExaminersMust have a valid US Driver's License. Ability to meet approved Melmark driver status.A minimum of one (1) year experience in related field preferred; however, over two (2) years experience is preferred.LPN Responsibilities
Maintains current and detailed knowledge of applicable regulations including documentation and maintenance of individual medical records.Participates in chart audits and chart reviews or other QA tasks to maintain quality of care.Communicates relevant healthcare concerns to physicians, appropriate healthcare staff, families and assists in coordinating treatment process.Maintains effective and regular communication with clients, families and caretakers.Responds immediately to Codes and medical emergencies with the appropriate equipment.Responds to work related incidents / accidents of employees. Provides evaluation, treatment, make appropriate referrals for treatment in accordance with established policies and procedures and ensures that the proper documentation is completed.Completes all nursing documentation as required. Reviews all nursing documentation for accuracy and makes updates to the nursing report.Assumes the primary care responsibilities for assigned clients including case management and client healthcare assessments.Provides case management in conjunction with other Melmark health care professionals and paraprofessionals.Benefits
