Location : White Earth Health Center, Ogema, MN & Nay-Tah-Waush Clinic (Mahnomen, MN)

Schedule : Monday–Friday, 8-hour shifts between 7 : 00AM–4 : 30PM (No overtime)

Summary

The Physical Therapist will provide comprehensive rehabilitation services in an outpatient clinic environment serving Native American patients across the lifespan—from infants to the elderly. This role independently evaluates and treats patients with orthopedic, neurologic, post-surgical, and chronic conditions, develops treatment plans, documents in the EHR, and participates in interdisciplinary care with clinic providers and medical staff.

Primary Duties

• Perform initial PT evaluations and establish individualized treatment plans
• Deliver direct hands-on physical therapy treatment for acute and chronic conditions
• Manage caseload of approximately 10 patients per 8-hour day
• Provide education to patients and families regarding exercises and functional mobility
• Collaborate with physicians and interdisciplinary providers to optimize outcomes
• Update EHR documentation for all patient encounters and progress
• Participate in orientation, on-site training, and mandatory IT / HIPAA security training
• Maintain a safe, clean, and professional clinical environment
• Adhere to clinic policies, safety standards, schedule protocols, and infection control
• Submit billing and credentialing documentation as required
