FTE : Part Time, 0.5, 40.00 hours per pay period (2 weeks)

Pay : $33.82 - $50.73 / hour. Pay is dependent on applicant's relevant experience

Summary :

This position is specifically for a Palliative Care Social Worker with the University of Colorado Hospital Palliative Care Service. Positions may include working in the inpatient, emergency department, and clinic settings. Assists in facilitating values-based goals of care discussions, advance care planning conversations, and transitions of care for palliative care service patients. Assesses patients and / or patient's family, caregivers, and / or legal representatives and arranges for needed interventions. Plans palliative care-focused interventions to help patients cope with physical, social, emotional, economic, and environmental concerns and provides short term treatment planning for grief support.
