  • This is a Grant Funded position with expected annual funding continued through September 29, 2027, pending annual SAMHSA approval.
  • LOCATION : Remote - this is a home based, virtual position that operates Monday-Friday from 8 : 30am-5 : 00pm. Candidate must live in or near Vaya's Catchment area. The person in this position must maintain residency in North Carolina or within 40 miles of the NC border. Must be willing to travel as needed.

    GENERAL STATEMENT OF JOB

    The System of Care High-Fidelity Wraparound (SOCHFW) Liaison is a staff position required by the NC Division of Child and Family Well-being's (DCFW's) System of Care - High Fidelity Wraparound Expansion Grant, funded by the Substance Abuse Mental Health Services Administration (SAMHSA). This four-year grant is expected to be funded, pending annual SAMHSA approval, through September 29, 2027.

    The SOCHFW Liaison serves as a liaison between Vaya's System of Care team and High-Fidelity Wraparound (HFW) providers. The SOCHFW Liaison collaborates with the DCFW's SAMHSA grant team to support local child collaboratives in results-based accountability (RBA) project development, implementation, evaluation, and utilization of grant funds in accordance with the DCFW's project requirements. The SOCHFW Liaison works in close collaboration with the SAMHSA grant's evaluation and implementation teams to ensure that the project's goals and objectives are implemented and evaluated to fidelity.

    ESSENTIAL JOB FUNCTIONS

    Oversight of SOC Collaborative Project Funds Program :

    • In collaboration with the DCFW's SAMHSA grant team, support local child collaboratives in developing and implementing community-based, results-based accountability (RBA) projects that support awareness and utilization of HFW teams.
    • Facilitate child collaboratives' annual applications for project funds and timely utilization of allotted funds.
    • In collaboration with the SAMHSA grant's evaluation team, support project-based data collection and reporting.
    • Development and implementation of strategies to increase the utilization of current HFW teams :

    • Work collaboratively with the HFW provider agencies to assess current programmatic strengths, needs, and gaps, and to determine potential intervention and support strategies.
    • Coordinate with UNC Greensboro to understand and educate others on standards and requirements regarding statewide HFW training, program structure, expectations, needs, etc.
    • Work collaboratively with internal Vaya teams, including SOC Coordinators, Family Partners, embedded Care Managers, Provider Network Operations, and others as needed, to enhance and ensure thorough, timely, and relevant communication between internal and external stakeholders regarding HFW teams, utilization, needs, successes, and strengthening opportunities.
    • Outreach to internal and external stakeholders, including caregivers, youth, family / youth support groups, child collaboratives, lived experience collaboratives, and other community members and groups as needed / requested, to provide education on HFW, as well as other service delivery systems identified as evidence-based practices in the SAMHSA grant (e.g., Trauma Focused Cognitive Behavioral Therapy; Multisystemic Therapy; Intensive In-Home Treatment).
    • Collaboration with Complex Systems Innovations (CSI) and SOC-HFW Liaison Cohort :

    • As part of a cohort of LME / MCO SOCHFW Liaisons, meet regularly, at a schedule determined by the DCFW's SAMHSA grant team, with the project's evaluation team to discuss statewide trends, observations, needs, intervention strategies, and successes.
    • In collaboration with the SOC-HFW cohort and CSI, determine annual action plans and sustainability strategies pertaining to project goals and objectives.
    • Reporting :

    • Complete and submit reports as requested by the SAMHSA Grant Project Director, in accordance with templates and requirements provided by the Project Director.
    • KNOWLEDGE, SKILLS, & ABILITIES

    • Thorough knowledge of High-Fidelity Wraparound and System of Care core values, principals, and practices
    • Ability to work effectively and collaboratively with a wide range of family members, stakeholders, community leaders, government agencies, providers, and other Vaya staff
    • General knowledge of family and community cultures, dynamics, and needs
    • Knowledge of resilience-building, trauma-responsive care, and results-based accountability
    • General knowledge of Department of Social Services, Department of Juvenile Justice, school, Health Department, and other service agency protocols
    • Must be able to maintain confidentiality and follow all agency policies and procedures.
    • Maintain a valid NC driver's license.
    • EDUCATION & EXPERIENCE REQUIREMENTS

      Master's Degree in a human service field and at least two (2) years of experience working in or with child public serving systems.

      PHYSICAL REQUIREMENTS

    • Close visual acuity to perform activities such as preparation and analysis of documents; viewing a computer terminal; and extensive reading.
    • Physical activity in this position includes crouching, reaching, walking, talking, hearing and repetitive motion of hands, wrists and fingers.
    • Sedentary work with lifting requirements up to 10 pounds, sitting for extended periods of time.
    • Mental concentration is required in all aspects of work
    • RESIDENCY REQUIREMENT : The person in this position is required to reside in North Carolina or within 40 miles of the North Carolina border.

      SALARY : Depending on qualifications & experience of candidate. This position is exempt and is not eligible for overtime compensation.
