Job Title : Field Care Manager, Behavioral Health

Location : Remote (Home Office) with 15–20% travel to Wayne or Macomb County, Michigan

Position Overview

Join our compassionate team and help us put health first. As a Field Care Manager, Behavioral Health , you will assess and support members to achieve optimal wellness by connecting them with appropriate resources. This role involves varied assignments and requires independent judgment.

You will work from a home office and travel locally (15–20%) to conduct in-home visits with Medicare / Medicaid members in Wayne or Macomb County, Michigan.

Key Responsibilities

• Manage a caseload and conduct assessments in-home, community-based settings, and telephonically.
• Collaborate with the Interdisciplinary Care Team (ICT) to support members with complex medical needs.
• Develop and coordinate care plans, address barriers to care, and advocate for member outcomes.
• Complete medical attestations and clinical oversight reviews.
• Ensure members receive services in the least restrictive setting.
• Modify Individual Care Plans and involve relevant care team members.
• Serve as the primary contact for the ICT and coordinate with internal and external resources.
