Health Center at Galloway is a skilled nursing facility, part of the Continuum network, providing clinically sophisticated sub-acute care and rehabilitation, as well as comprehensive nursing care. With extensive leadership experience in the nursing, sub-acute, and rehabilitative field, Health Center at Galloway has the resources and capabilities to provide the highest quality care through a full range of services.

We are seeking a compassionate and skilled Certified Social Worker (CSW) to join our team in providing vital social work services to residents and their families. The CSW will play an integral role in supporting residents' emotional, social, and psychological needs while promoting their overall well-being within the long term care setting.

Job Summary

Duties include:

• Care planning

• Discharge planning

• Advocating for residents’ rights

• Providing emotional support to residents and their family members.

Qualifications:

• Must possess a bachelor’s degree in a human service field

• Must have long term care, Point Click Care and MDS experience

• Must be Certified Social Worker in New Jersey

• Experience working in a nursing home setting preferred

Preffered experience:

• Nursing Home Social Service: 2 years preferred

Required education:

• Bachelor's degree
