Texas Licensed Clinical Navigator

A company is looking for a PT Clinical Navigator.

Key Responsibilities

Conduct clinical intake assessments and gather information to evaluate patients' conditions and treatment needs

Build trust-based relationships with patients and provide education on treatment options and care plans

Monitor patient progress and address barriers to care while ensuring continuity and coordination of care

Required Qualifications

Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required

Current, unrestricted PT license in Texas (Compact License preferred)

Minimum 2 years of relevant experience in physical therapy or MSK-related roles

Willingness to obtain additional licensure in new states as needed

High comfort level with technology and data-informed clinical workflows
