Nevada Licensed Disease Management RN

A company is looking for a Disease Management RN - Bilingual (Spanish).

Key Responsibilities

Make outbound and receive inbound calls to assess members' health status

Identify gaps in treatment plans and provide patient education for self-management

Support members with condition education and connect them to resources

Required Qualifications

Current, unrestricted Nevada RN license or ability to obtain one

3+ years of experience in a hospital or direct care setting

Bilingual in Spanish

Background in diabetes management

Ability to work remotely and independently
