The Recovery Support Navigator is responsible for Case Management, Network Support Facilitation, and Case Coordination within our community-based initiative.
Salary : $17.50 / hour with productivity incentives.

Duties & Responsibilities
Meet with client face to faceDevelop a set of goals and objectives identified by clientImplement goals and objectives in comprehensive care plan / family support planDeliver services to client in the community (home, treatment facility, diversionary unit)Obtain insurance authorizations and conducts concurrent reviewsAssist client with transportation needsArrange referrals for services when neededCoordinate care with inpatient staff, aftercare facilities and community partnersProvide linguistically appropriate and culturally sensitive recovery support navigationDocument all services in electronic health recordParticipate in program meetings and supervision sessionsShare 24 / 7 on callUtilize Arise model with clients and their families when applicableEducation & Experience
Bachelor's Degree in a related field preferred, but will consider lived experienceMust have own transportation, valid license, and proof of insuranceStrong evidence of effectiveness in engaging clientsSolid Knowledge of SSTAR services as well as community resources
