Family Practice - Without OB Physician

We are seeking a Family Medicine Physician to join our innovative care team in West Point, GA. Deliver whole-person care in 30- to 60-minute appointments, supported by a health coach and care team, focusing on high engagement, managed care, and culture transformation to reduce costly service utilization and improve overall health outcomes.

Position Highlights : See 10-15 patients daily in longer, meaningful appointments. Collaborative, team-based approach with health coaching support. Focus on proactive, value-based care

Requirements :

• MD or DO degree with completed Family Medicine
• Board Certified
• At least 3 years of primary care experience
• Current, valid medical license and DEA number
• Proficiency with EMR systems (NextGen preferred), coding, and Medicare Advantage / HCC documentation
• Experience with Google Workspace and team-based care
