A supportive employee-owned home care organization in Northern New York is seeking a dedicated Physical Therapist to provide in-home care to adult patients in the Saranac Lake area. This opportunity offers flexibility autonomy and the ability to make a meaningful impact while enjoying a balanced work-life schedule. Full-time or part-time options are available.

Responsibilities :

Evaluate patients using diagnostic muscle nerve joint and functional ability assessments.

Develop individualized treatment plans and modify care as needed in coordination with physicians.

Provide direct home-based therapy focused on improving mobility function and independence.

Educate patients and caregivers on home exercise programs and use of assistive devices.

Maintain accurate up-to-date clinical documentation for all visits.

Participate in interdisciplinary case conferences staff meetings and training opportunities.

Ensure patient safety and proper use of equipment during care.

Qualifications :

Graduate of an accredited Physical Therapy program.

Current and valid Physical Therapy license issued by the New York State Department of Education.

Minimum of one year of experience as a licensed physical therapist.

Strong interpersonal assessment and problem-solving skills.

Ability to work independently and manage a home visit schedule efficiently.

Compensation and Benefits :

Annual salary range : $80000 $100000 based on experience.
