Position Overview
Quality Healthcare Group, a leading provider of home and community-based services, is seeking a Lead Care Professional & Head Trainer—a highly capable, motivated individual who will not only provide direct care but also oversee training, onboarding, and competency development for all new CNAs, HHAs, and Direct Care Workers.
This role requires elevated responsibility, strong judgment, and the ability to model exemplary care standards. You will serve as a primary resource for new staff, ensuring that all care delivery aligns with ODP expectations, QHCG policies, and individualized client needs.
We specialize in serving individuals with intellectual disabilities, developmental disabilities, and those receiving services under Pennsylvania’s Office of Developmental Programs (ODP). Experience with individuals with Traumatic Brain Injury (TBI) is strongly preferred.
The Lead Care Professional plays a critical role in helping clients maintain independence, safety, and quality of life—and in ensuring QHCG staff are fully prepared to deliver exceptional care.
Direct Responsibilities
Assist clients with activities of daily living (ADLs) : bathing, dressing, grooming, toileting
Provide safe mobility support, transfers, and appointment-related transport assistance
Prepare meals and support feeding when indicated
Perform light housekeeping to maintain a safe and sanitary environment
Engage clients in recreational, therapeutic, or community-based activities
Monitor and promptly report changes in behavior, health status, or safety concerns
Maintain accurate, timely documentation and adhere to individualized service plans
Follow all QHCG policies, ODP regulations, and applicable care standards
Head Trainer Responsibilities
Serve as the primary trainer for all new CNAs, HHAs, and Direct Care Workers
Conduct onboarding training on :
ADL protocols
TBI- and IDD-specific care
Behavioral supports
Abuse / neglect prevention
Mandated reporting
Documentation standards
ODP-required competencies
Oversee skills check-offs and ensure staff demonstrate competency before independent assignment
Provide ongoing coaching and corrective guidance to staff as needed
Assist with the development and refinement of care procedures, workflows, and training materials
Conduct periodic in-field observations to reinforce care quality and identify training gaps
Serve as a point of escalation for staff seeking clarification about care tasks or ODP expectations
Coordinate with supervisors to ensure all new hires meet regulatory training timelines
Uphold and model QHCG’s standards for professionalism, communication, and client-centered care
Qualifications
CNA Certification, HHA Certification, or minimum 1 year of direct care experience required
Leadership or trainer experience strongly preferred
Experience with ODP programs is a significant plus
Experience working with clients with TBI highly preferred
High school diploma or GED required
Strong communication, coaching, and leadership skills
Ability to model excellent care and teach others effectively
Reliable transportation; valid driver’s license highly preferred
Must pass all background checks, health screenings, and onboarding requirements
