Seeking a compassionate and skilled Registered Dietitian to join a patient-focused team supporting individuals with End-Stage Renal Disease (ESRD). In this role, you will make a direct impact on patients' health and quality of life by delivering tailored nutritional assessments, plans, and ongoing support in a dialysis setting.
Key Responsibilities :
Conduct comprehensive nutritional assessments and develop individualized care plans
Monitor lab results and adjust diets / medications in coordination with care teams and protocols
Collaborate with physicians and interdisciplinary teams to optimize patient outcomes
Provide education to patients and families on renal diets and wellness strategies
Participate in quality improvement initiatives and care plan meetings
Maintain HIPAA compliance and professional communication across all interactions
Qualifications :
Registered with the Commission on Dietetic Registration Required
Current state licensure Required
Minimum 1 year of clinical nutrition experience post-registration Required
Experience with ESRD patients Preferred
Member of the Academy of Nutrition and Dietetics Preferred
