Your primary responsibilities will include :

• Conduct comprehensive OT assessments to identify functional limitations and participation barriers; - Develop and implement individualized intervention plans focusing on activities of daily living, mobility, cognition, sensory integration, and community reintegration; - Set measurable goals, track progress, and adjust plans in collaboration with clients, caregivers, and the broader care team; - Document outcomes in electronic health records, ensuring compliance with best practices and payer requirements; - provide client and family education, equip them with strategies for home programs, and facilitate discharge planning and community referrals.
