MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve. This position offers both variety and rewarding challenges as you lead clinical care efforts and ensure the well-being of those we serve. Responsibilities :

• Provide direct nursing care, including medication administration, wound care, health assessments, and coordination of medical appointments
• Supervise and support a team of RNs and / or LPNs, including hiring, onboarding, scheduling, and ensuring regulatory compliance
• Act as a clinical resource and role model, offering guidance on nursing best practices and patient-centered care
• Monitor and manage the overall health and medical needs of individuals served, ensuring timely and appropriate interventions
• Conduct and document evaluations of nursing care and staff performance to maintain a high standard of care
• Assist with development and implementation of individualized care plans
• Ensure a safe, clean, and comfortable environment by maintaining infection control and safety protocols
• Collaborate with interdisciplinary teams, families, and external providers to ensure continuity of care Qualifications :
• Graduate of an accredited nursing program with current RN license
• Strong leadership and communication skills
• Excellent organizational abilities with the capacity to multitask and meet deadlines
