Overview :
Outpatient Coding Abstractor
- Codes ambulatory surgery and outpatient IVR accounts.
- Reviews clinical documentation and diagnostic results as appropriate to extract data and apply appropriate ICD-10-CM, CPT-4, HCPCS and modifier codes for hospital reimbursement, medical necessity, mortality & morbidity outcomes, research, statistical information and regulatory compliance.
- Accurately codes ambulatory surgery and outpatient IVR encounters diagnosis and procedures supported by the health record documentation in accordance with ICD-10-CM Official Coding Guidelines for Coding and Reporting. Abstracts, codes and assigns necessary demographic and clinical data elements required for outpatient records.
- Effectively utilizes encoder software to ensure appropriate reimbursement and accurate APC assignment. Completes all work in accordance with defined productivity and quality standards. Reviews charge capture of complex cardiac procedures and other IVR procedures for accuracy.
- As necessary queries physician for additional information or clarification of diagnoses, co-morbid / secondary conditions, and procedures.
- Assures optimal ethical reimbursement for coded encounters and assures coding practices fall within established compliance guidelines. Performs other related duties as requested by HIM management.
Certified Professional Coder (CPC), Certified Interventional Radiology Cardiovascular Coder (CIRCC), Certified Inpatient Coder (CIC), Certified Outpatient Coder (COC) or Certified Coding Specialist (CCS) or Registered Health Information Technician (RHIT) required .
Knowledge of medical terminology and anatomy & physiology required .
