Nurse Practitioner NP

Qualifications

Active NP license in New Jersey

Minimum 6 months of NP experience — ICU, ED, or hospitalist experience is a strong plus

Strong clinical judgment and ability to manage medically complex populations

Excellent communication and documentation skills

Responsibilities

Provide medical management for both subacute and long-term care residents

Conduct assessments, manage acute changes, and follow chronic conditions

Document all encounters timely and accurately

Communicate closely with nursing, therapy, social services, and administrative teams

Serve as a consistent clinical presence and leader within the building
