Director of Case Management (RN)

Clinical Magnet (Part of ICON Medical) is on the lookout for a passionate and experienced Director of Case Management (RN) to lead a skilled team at our partner's rehabilitation hospital, a 70-bed facility, in the Tallahassee, FL area. This is a unique opportunity to take on a pivotal leadership role where you can influence patient care delivery and enhance recovery outcomes.

Your Responsibilities :

  • Manage the daily operations and personnel of the Case Management department.
  • Direct interdisciplinary care planning and discharge processes to achieve the best possible patient results.
  • Act as an advocate for patients and families while fostering strong relationships with payers and healthcare stakeholders.

    • What You Bring to the Role :

  • Active RN license in Florida and a BSN or higher.
  • Minimum of 3 years of hospital-based Case Management experience (acute care, inpatient rehab, or LTACH).
  • 2+ years in a leadership role (supervisor level or above).
  • Exceptional communication, organizational, and collaborative skills.
