As an integral member of our interdisciplinary team, you will play a vital role in ensuring that patients and their families have the guidance and resources they need to navigate the hospice journey with dignity and care.

As a Hospice Social Worker (LMSW), Your Responsibilities Will Include :

• Serving as the psychosocial support expert on the interdisciplinary care team.
• Conducting assessments and care planning tailored to the emotional, psychological, environmental, and cultural needs of patients and families.
• Providing counseling, crisis intervention, and practical support aligned with each patient's individualized care plan.
• Collaborating closely with patients, families, physicians, nurses, chaplains, and other team members to promote the emotional and social well-being of the patient-family unit.
• Educating families about hospice services, advance care planning, grief, and coping strategies.
• Assisting with long-term planning, community referrals, and resource navigation as needed.
• Monitoring and documenting psychosocial changes in the patient or family situation and recommending updates to the plan of care when appropriate.
• Upholding ethical standards and exercising professional judgment in end-of-life care.
