Job descriptionAssess and identify patients’ and families’ physical, psychosocial, environmental, safety, and developmental needs. Develop and individualize patient care plans based on assessments. Document problems, nursing assessments, goals, interventions, and outcomes in the EMR. Assess pain and develop individualized pain management plans. Anticipate, prevent, and treat undesirable or secondary symptoms. Participate in interdisciplinary team (IDT) meetings to review and update plans of care. Educate patients, caregivers, and volunteers on interventions, medications, and side effects. Provide compassionate end-of-life care, supporting comfortable and dignified dying. Support patients’ and families’ spiritual and cultural needs. Coordinate patient and family services, ensuring continuity of care. Maintain accurate, complete, and compliant electronic medical records. Collaborate with the care team to ensure quality hospice and palliative care delivery.
