• Home health visits with travel to multiple cities in Vermont
• Active Vermont license required
• Monday-Friday, (5x8 or 4x10)
• EMR : 360
• Min of two years of working experience preferred
• Experience with in-home care preferred
• Population health management experience preferred
• We provide complimentary housing and travel
