Description :

Rehab experience preferred.

Three references must be provided.
County criminal background check for seven years for all counties lived and worked is needed.
Federal criminal background check for seven years or more is required.
Multi-state criminal search for seven years or more is necessary.
Nationwide sex offender search for seven years or more is required.
HHS / OIG search is necessary.
GSA search must be conducted.
Social security number trace is needed.
2 Step PPD or Quantiferon or proof of positive PPD and chest X-ray is required.
SAM search is necessary.
Washington State background with fingerprints is required for facility run positions.
