Job Summary and Responsibilities

Functions independently and consistently in the delivery of dialysis therapy to all types of patients including those in complex situations with overt and covert needs and / or complications of treatment through

assessment planning implementation and evaluation. The registered nurse will be responsible to prescribe delegate and coordinate nursing care. This position requires providing care / service from a chronically ill to a critically ill through rehabilitating adolescent through geriatric patient population in a manner that demonstrates the functional / developmental age of the individual served. The registered nurse will incorporate multiple disciplinary care and management with involved health care team members.

• Adheres to established Hospital / departmental policies and procedures objectives Quality Assessment and Improvement Program legal safety environmental infection control standards Nurse Practices Act and Patients Bill of Rights.
• Applies clinical knowledge and expertise for coordinating patient care by regularly planning and evaluating the care of patients.
• Establishes and maintains professional and collaborative relationships with other staff members physicians patients patient family members and the public as appropriate while assuring confidentiality of all patient information.
• Demonstrates effective nursing judgement and a working knowledge of the nursing process.
• Provides guidance to interdisciplinary team members by supervising nursing interventions and assisting with related tasks.
• Provides leadership by establishing priorities and coordinating nursing care.
• Documents and communicates all observed symptoms reactions treatments and changes in the patient condition to the appropriate member of the interdisciplinary health care team.
• Provides for teaching based on identified need.
• Coordinates plan of care with other members of the interdisciplinary health care team to achieve desired goals and outcomes.
• Communicates the plan of care to the patient and care partner(s) for the active involvement of the patient in the evolution of the plan throughout hospitalization.
• Conforms to legal requirements of nursing practice and Hospital policies and procedures.
• Administers medications to assigned patients as directed and assesses and documents patient responses; initiates appropriate interventions per Hospital / Unit policy.
