Responsibilities :

• Provide hands-on nursing care and support for patients with chronic conditions like diabetes, HIV, hypertension, and obesity.
• Coordinate care for elderly and disabled individuals, ensuring they receive the attention and resources they need.
• Travel between multiple care sites within Fairfield County (no overnight travel required).
• Work closely with a multidisciplinary team to deliver comprehensive, patient-centered care.
• Manage referrals and assist patients in navigating their healthcare journey.

Pay and Benefits

The pay range for this position is $33.65 - $36.06 / hr.
