About the Role
We’re seeking a compassionate and mission-driven Behavioral Health Clinician to join a dynamic, community-focused team. This role is ideal for a clinician passionate about advancing health equity, integrating behavioral health into medical care, and supporting patients managing both mental and physical health challenges.
As a key member of a multidisciplinary care team, you’ll work directly with individuals in the community who are at high risk for cardiometabolic and behavioral health conditions such as diabetes, hypertension, obesity, depression, and anxiety. This is a unique opportunity to make a measurable impact on the well-being of underserved populations.
Key Responsibilities
- Provide integrated behavioral health care for patients with cardiometabolic and mental health conditions.
- Conduct psychosocial assessments, develop treatment plans, and deliver short-term therapy (up to six months).
- Collaborate with primary care and behavioral health providers to support patients’ self-management goals.
- Use evidence-based tools (e.g., PHQ-9) to monitor and evaluate treatment outcomes.
- Lead and facilitate group sessions for behavioral health and chronic disease management.
- Participate in community outreach programs and educational events focused on wellness and prevention.
- Attend regular case reviews, team huddles, and interdisciplinary meetings.
- Maintain accurate, timely clinical documentation and uphold confidentiality standards.
- Incorporate trauma-informed and culturally responsive care principles in all patient interactions.
Qualifications
Education : Master’s Degree in Social Work or Mental Health Counseling (Required)Licensure : Active LMHC or LICSW license in Massachusetts (Required)Languages : Fluency in Haitian Creole or Spanish (Required)Experience :Proven experience providing individual counseling within the communityStrong background working with diverse populations in community-based settingsSkills :Excellent communication and collaboration skillsComfort working in a fast-paced, multidisciplinary environmentCultural competence and sensitivityProficiency with electronic health records and Microsoft Office toolsCompensation & Benefits
