Assignment Dates : As soon as possible - Ongoing (1 Day per week)

Schedule / Shifts : Flexible 1 day per week, Monday to Friday, 8 AM to 5 PM

% inpatient / outpatient : 100% outpatient

Patient Ages : All ages up to 18 years

Patient Volume : Varies day to day based on procedure mix

Procedures : General pediatric dentistry

Support Staff : Front Office, 3 dental assistants, 3 hygienists, 1 other dentist

Type of Charting : Open Dental

Requirements : Oregon dental license, DEA registration, and BLS

Reason for Need : Growth, looking for locums or long-term

Credentialing Timeframe : 1 week

Pay Rate : Negotiable
