Shift : MondayFriday 85 with rotating weekends and on-call duties

Job Description :

Sunbelt Staffing is seeking a skilled Home Health RN in Rock Hill, SC. The RN will conduct in-home patient visits, complete OASIS documentation, and manage care plans through Homecare Homebase (HCHB). This role is ideal for a nurse who values autonomy and delivering compassionate care directly in patients homes.

Key Responsibilities :

Perform comprehensive OASIS assessments for admissions, recerts, discharges, and resumptions

Provide direct nursing care, medication management, and patient / family education

Accurately document care in Homecare Homebase (HCHB)

Coordinate with physicians and the care team to ensure optimal patient outcomes

Ensure quality and compliance with agency and Joint Commission standards

Qualifications :

Active RN license in South Carolina

Minimum 2 years of nursing experience, including at least 1 year of home health

OASIS and Homecare Homebase (HCHB) proficiency required

Reliable car, valid drivers license, and insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
