As a Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social screenings, care coordination and member education.

Travel 50% in Buncombe County, NC , NC and surrounding areas within 120 miles.

Schedule Monday-Friday standard business hours. No nights, no weekends and no holidays. We offer our established staff the option to work 4 10-hour days (7 am-6 pm) in lieu of the traditional 8 hour 5 / day week schedule.

The hourly pay for this role will range from $20.00 to $35.72 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
