Job Schedule : Occasional / PRN

We are seeking a MDS Coordinator to join our support team, committed to delivering outstanding service to our employees and residents. As a MDS Coordinator, you will play an important role in conducting and coordinating the development and completion of the Resident Assessment Document, CAAS and care planning that meets or exceeds Buckner quality, service, and hospitality standards. What you'll do :

  • Input data onto the RAI instrument either via computer or long-hand in a manner that meets federal and state requirements. Complete CAAs, Care planning and transmit MDSs through designated portal on a timely basis.
  • Review clinical charting - to ensure accurate data can be reflected on MDS and care planning.
  • Perform transmissions of the MDS on a timely basis.
  • Assures compliance with Federal and State regulations regarding completion / coordination of the comprehensive assessment (MDS).
  • Assist with ABN's, Physician certification and recertification as needed if working on site.
  • Assists in Managed Care authorization process.
  • Monitors MDS and care plan documentation on all residents, assuring that complete, orderly chronological and current documentation is in file.
  • Conduct in-service training for certified and licensed associates as needed regarding the MDS procedure and requirements.
  • Provide the Nurse Manager- DON / ADON a list of charting windows as needed for upcoming assessment review dates
  • Notify the Nurse Manager- DON / ADON immediately of the issues noted upon review of the chart while doing the MDS. Make daily contact with the Nurse Manager- DON / ADON) immediately to discuss any issues which might impact the MDS process.
  • Notify the Director of Nursing of any issues relevant to the functioning of the nursing units which may negatively impact patient care.
  • Provide DON, Administrator and pertinent staff with the QI / QM report and coordinates access to software for team members to access own reports.
  • Remains current with changes in the industry.
  • Performs care plan updates as needed.
  • Coordinate with the Administrator, Director of Nursing, and Quality Assurance personnel regarding issues noted on the Facility Quality Indicator Profile.
  • Obtain weekly therapy updates from Therapy Department for required MDS documentation.
  • Attend Quality Assessment and Assurance Committee meetings, Triple Check meetings, Care Plan meetings, and other meetings as requested.

What you'll bring :

  • Requires basic understanding of a specialized area of knowledge normally acquired through four years of high school, plus two to three years of college and appropriate license / certification. Minimum 1 year prior related experience in long term care preferred.
  • Requires working knowledge of general nursing theory and practice, including basic knowledge related to nursing, such as drugs, biological, physical, social, and medical sciences, and their application.
  • Requires working knowledge of infection control, universal precautions and OSHA standards.
  • Must be a graduate of an accredited school of nursing. Must hold and maintain a current Texas licensure as a Registered Nurse (RN) or Licensed Vocational Nurse (LVN). Must complete and / or meet training and certification required by Buckner and the State of Texas.
  • Ability to travel as needed to our BRS locations to support MDS operations.
