We are hiring a BSAS/SOAR Case Manager!

The BSAS/SOAR Case Manager role involves assessing client needs, developing treatment goals, facilitating groups and youth programs, and connecting clients and families with resources to support long-term success. The BSAS/SOAR Case Manager also ensures accurate documentation, maintains confidentiality, and fosters strong relationships with community partners, schools, and families.

• To work as an early intervention specialist with clients who have been identified at risk for substance use disorders.
• To provide comprehensive care coordination of services for clients at assigned location.
• To assess their client’s overall mental status
• To coordinate treatment goals.
• To connect clients and their families with the resources they need to successfully manage their conditions on their own and engage parents.
• To transport, as needed, clients to community events and services.
• To provide or refer clients to tutoring and homework assistance

Required:

• Bachelor’s Degree in counseling, psychology, social work or related field.
• Valid MA Driver’s License
  
• Prefer 1-year experience working with Substance use populations
