Palliative Care Provider

This is a career-defining role for a highly autonomous Palliative Care Physician or Palliative Care Nurse Practitioner who is passionate about delivering compassionate patient-centered palliative care. You will be instrumental in optimizing the quality of life for our most vulnerable members utilizing your expertise to manage complex symptoms drive crucial advance care planning and ensure dignity through seamless timely transitions to hospice care.

As a Palliative Care Provider you will :

• Provide comprehensive palliative care to patients in their homes focusing on pain and symptom management improving quality of life and offering emotional and spiritual support.
• Perform in-depth assessments of patients with serious or life-limiting conditions develop personalized care plans and manage their medical needs.
• Work closely with patients and their families to establish goals of care and ensure treatment aligns with their wishes emphasizing patient autonomy and dignity.
• Work in collaboration with a multidisciplinary team (nurses social workers and other healthcare professionals) to ensure a holistic approach to patient care.
• Participating in a shared on-call rotation as directed by the Senior Medical Director.

A reasonable estimate of the base salary range for Physicians is $240000 to $300000 and for NPs is $145000 to $165000. Final pay is based on several factors including but not limited to internal equity market data and the applicants education work experience certifications etc.
