The Riverside Premier Rehabilitation and Healing Center is looking for a talented and hard-working Social Work to join our ever-growing team.

The Riverside Premier Rehabilitation and Healing Center is a facility with 520 beds and is proud of their state-of-the-art Rehabilitation Gym servicing 150 + short term beds. The Riverside Premier Rehabilitation and Healing Center team consists of highly dedicated clinicians that serves sub-acute and long-term residents in a excellent setting for learning and growth. The Riverside Premier Rehabilitation and Healing Center provides Rehabilitation services 7 days per week.

Social Worker Responsibilities Include:

Assumes authority, responsibility, and accountability for the delivery of medical related social services in the facility. Collaborates and coordinates in an efficient and economic sound process with the interdisciplinary team

• Identifying individual social and emotional needs

• Developing and maintaining individualized care plans

• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility

• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay

• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Social Worker Qualifications/Requirements:

• BSW or MSW preferred, or Bachelors degree in related field

• At least one year experience in Social Work field required

• Long term care and/or sub-acute setting required

• Ability to work flexible schedule

• PCC experience preferred

• Knowledge of MDS 3.0 preferred

Salary range : $34 - $36. per hour
