• Duration : 13-week contract with possibility of extension upon successful completion\n
• Location : Jefferson, IN, and surrounding areas\n
• Type : Full-time, Scheduled 40 hours per week - no guaranteed hours\n
• Schedule : Monday to Friday, 8 : 00 am to 5 : 00 pm, with occasional weekends and on-call duty (typically one weekend per month)\n
• Productivity Expectations : 30-32 per week, up to 35 visits\n
• Compensation : Hourly rate negotiable based on experience and territory budget, with weekly pay and .48c mileage reimbursement visit to visit, 150-250 miles per week expected\n
• Benefits : Medical, dental, vision, and 401k options available after 90 days.\n\nRequirements : \n
• Active Registered Nurse (RN) License in Indiana.\n
• Must be proficient in Oasis and Homecare Homebase.\n
• Minimum of 1 year of home health experience, including proficiency in Oasis and Homecare Homebase.\n
• Active American Heart Association or American Red Cross BLS certification.\n
• Demonstrated ability to work independently and as part of a team in a fast-paced environment.\n
• Excellent communication and interpersonal skills.\n
• Compassionate and patient-centered approach to care delivery.\n
• Ability to adapt to changing priorities and multitask effectively.\n
• Reliable transportation.\n
• Joint Commission onboarding standards upon offer.\n\nJob Duties : \n
• Conduct comprehensive assessments of patients' health conditions utilizing Oasis and Homecare Homebase software, ensuring accurate documentation and compliance with regulatory standards.\n
• Provide skilled nursing interventions, including wound care, medication management, and IV therapy, while prioritizing patient safety and comfort.\n
• Monitor patient progress closely, evaluating response to treatment and adjusting care plans as necessary to optimize outcomes and promote well-being.\n
• Educate patients and caregivers on disease management, medication administration, and symptom recognition, empowering them to actively participate in their care and promote self-management.\n
• Maintain meticulous documentation of all patient interactions, assessments, and interventions, adhering to agency policies and procedures and regulatory requirements.\n
