Job Description
PRIMARY PURPOSE :
To contribute to the Essential Services (ESS) team by providing comprehensive housing-related assistance to clients and managing short to long-term cases within the general housing Program, focusing on developing personalized action plans, conducting thorough needs assessments, and coordinating with local landlords and housing agencies to secure stable housing options. To serve as a dedicated advocate for low-income individuals and families who are at risk of homelessness, currently experiencing homelessness, or facing mental health and other challenges, ensuring their access to necessary resources and support by navigating complex social service systems, facilitating connections to healthcare providers, employment services, and educational opportunities, and offering ongoing case management to promote long-term stability and self-sufficiency. This role involves collaborating with multidisciplinary teams, maintaining accurate and detailed client records, participating in community outreach events, and staying up to date on relevant policies and best practices in veteran support services.
