We empower people to break the cycles of homelessness, welfare dependency, substance abuse, and incarceration through innovative work and housing programs.

We are seeking a Case Manager / Substance Abuse Counselor for The Doe Fund's newest permanent supportive housing program for homeless men and women living with HIV / AIDS. The program is designed to provide long-term residential care and twenty-four hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. Forty-one units will be allocated for residents referred by the New York City HIV / AIDS Services Administration (HASA) and twenty-seven apartments will be for adults referred by the New York City Housing Authority (NYCHA).

The Case Manager / Substance Abuse Counselor is responsible for providing supportive case management services to the HASA-funded residents. The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV / AIDS population.

RESPONSIBILITIES :

Maintain a caseload and provide regular case management services to these individuals

Maintain resident records in compliance with agency and funding standards

Conduct and review assessments

Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self sufficiency

Monitor and ensure residents are linked to healthcare providers

Provide counseling and crisis intervention as necessary

Assist residents in securing and maintaining benefits

Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA and mental health programs

Facilitate groups and activities that promote health and independent living skills development

Monitor and assist residents with rent payments

Conduct regular unit inspections

Compile resources on substance abuse

Educate residents and staff about approaches to managing addiction

Oversee recovery readiness, relapse prevention, and recovery oriented services

Organize sober recreational activities

Attend ongoing trainings to enhance the services provided to the residents

Assist with completion of HASA’s weekly and monthly reports as well as any other relevant reports

Other duties as assigned

QUALIFICATIONS :

Bachelor’s Degree in Social Work or relevant field

Credentialed Alcohol and Substance Abuse Counselor(CASAC) certification or a License in Clinical Social Work (LCSW) is strongly preferred

Ideal candidate must have at least one year of experience working with people who are homeless, living with HIV / AIDS, have substance abuse histories or suffer from mental illness

At least 3 years of experience in substance abuse counseling

Knowledge of benefits in the New York City Metro area, HIV / AIDS, psychiatric diagnoses, substance abuse, and mental health issues

Solid computer skills – internet, Microsoft Office, and charting database

Strong organizational, writing, and communication skills

Spanish speaking a plus, but not necessary
