Job Summary :

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team to do Oasis assessments and Visits , providing direct patient care, assess patient health needs, and develop care plans in coordination with physicians and other healthcare professionals.

Key Responsibilities :

• Conduct initial and follow-up patient assessments in the home setting.
• Develop, implement, and revise individualized care plans.
• Administer medications and treatments as prescribed.
• Educate patients and their families about their care plans, medications, and lifestyle adjustments.
• Monitor and document patient progress in accordance with agency policies and regulatory guidelines.
• Coordinate with physicians, therapists, and other healthcare professionals to ensure optimal patient care.
• Adhere to infection control and safety protocols.
• Participate in on-call rotations as required.

Qualifications :

• Active Registered Nurse (RN) license in Florida.
• Minimum of 2 years of nursing experience (home health experience preferred).
• Current CPR certification.
• Valid drivers license and reliable transportation.
• Strong organizational and communication skills.
• Ability to work independently and make sound clinical decisions.
