Role Description

We are seeking a compassionate and skilled Nurse Practitioner (NP) specializing in geriatric care, dementia care, and palliative care to provide comprehensive patient care both in-person with home visits in the community and assisted living facilities, and via telehealth. The ideal candidate will demonstrate a strong commitment to improving the quality of life for patients and their families while managing complex health issues related to the complex needs of geriatric populations. This role, based in Syracuse, NY, offers flexibility with a combination of in-person and telehealth responsibilities, allowing the NP to provide high-quality care to patients in various settings. The successful candidate will demonstrate a commitment to patient advocacy, teamwork, and comprehensive care delivery.

Qualifications

1. Education and Certification :

• Master's degree in Nursing (MSN) from an accredited program.
• Active Nurse Practitioner (NP) license in the state of practice.
• Certification in a relevant specialty, such as Adult-Gerontology Primary Care or Family Nurse Practitioner.
• Additional training or certification in gerontology, dementia care, wound care, and palliative care preferred.
